Computer vision syndrome is the cluster of eye and vision problems that shows up after prolonged screen work — blur, burning, headache, dry or watering eyes. It isn't a disease of the eye. It's a set of symptoms produced by a visual task the eye was never built to hold for eight hours.

Only asthenopia carries its own ICD-10-CM code (H53.14, visual discomfort); computer vision syndrome and digital eye strain don't. A clinician who writes either term names a presentation, then codes what sits underneath it: dry eye, an accommodative or vergence disorder, an uncorrected refractive error.
Computer vision syndrome symptoms, sorted by mechanism
The dominant group points to the treatment.
Ocular surface. Burning, grittiness, redness, contact lens intolerance, and — counterintuitively — watering. The driver is tear film instability: during concentrated screen work the blink rate drops and more of the remaining blinks are incomplete, leaving the lower cornea exposed. A high-mounted screen compounds it — raising your gaze widens the lid opening and speeds evaporation.
Accommodative and vergence. Intermittent blur at the screen, slow refocusing when you look away, brow ache building through the afternoon, occasional doubling — the focusing and eye-teaming system fatiguing under sustained intermediate demand.
Referred musculoskeletal. Neck and shoulder pain sits inside most CVS definitions rather than beside them, because the posture is usually a response to the visual problem — a head craned toward small text, a presbyope tipping their chin up to reach the bifocal segment.

How an optometrist assesses it
History comes first: hours per day, how many screens, the measured distance to each, and whether symptoms clear over a screen-free weekend. Symptoms that track the task point one way; symptoms present on waking point elsewhere. A workup then includes:

- Binocular vision testing. Near point of convergence, phorias measured at the actual screen distance, accommodative facility with ±2.00 D flippers. Convergence insufficiency is common and easily missed on a routine sight test.
- Tear film assessment. Tear break-up time — the gap between a complete blink and the first dry spot in the film — with under about 10 seconds generally read as unstable, plus surface staining and meibomian gland evaluation.

1. Scheduled visual breaks and blink retraining. First-line everywhere. 2. Workstation ergonomics. Viewing distance 50–70 cm, with the screen's center roughly 15–20° below horizontal eye level so your lids cover more of the cornea. Raise text size instead of leaning in, match brightness to the room, kill reflections, and stay clear of vents and fans.
3. Ocular surface management. Lubricating drops, ambient humidity, and lid hygiene where meibomian gland dysfunction is present. Dry eye has subtypes that are managed differently, so this is a clinician's call.
4. Optical correction. Correcting what working-distance refraction turns up is often the largest single change available. For presbyopes that usually means single-vision intermediate lenses or an occupational progressive — general-purpose progressives squeeze the intermediate zone into a narrow channel that forces constant head movement at a monitor. Convergence insufficiency may be managed with prism or vision therapy.
5. Where the evidence is weaker than the marketing. Blue-light-filtering lenses are the clearest case: systematic reviews have not found a reliable short-term benefit for screen-related eye strain, and their sleep effects are a separate question with its own mixed literature.
